We require the following information for all new pets:

• Pet Name
• Pet Breed, Age, & Sex
• Parent Name
• Phone Number
• Address
• Email
• Preferred Vet Contact 

All dogs must have the following vaccinations before grooming: 

• Rabies
• Distemper/ Parvovirus
